Abstract: | 隨著高齡社會與失智症人口增加,失智症家庭照顧者所面臨的情緒困擾與心理健康問題日益嚴重。本研究旨在探討以接納與承諾治療(Acceptance and Commitment Therapy, ACT)為理論架構之團體方案,對失智症家庭照顧者進行團體諮商介入,評估其在心理彈性與價值導向行動的促進效果上的效益。
本研究採用三梯次多基線跨團體設計(Multiple Baseline Across Groups Design),共招募19位失智症家庭照顧者,接受為期五週、每週一次的ACT團體介入,課程設計融合世界衛生組織壓力因應手冊《Doing What Matters in Times of Stress》之核心元素。研究工具包括接納與行動問卷第二版(AAQ-II)、價值問卷(VQ)、病人健康狀況憂鬱問卷(PHQ-9)與廣泛性焦慮量表(GAD-7),於團體介入前後施測,以評估參與者接受團體介入後在心理彈性與價值行動之變化。
研究結果顯示,實驗介入組在AAQ-II得分方面顯著下降(M = -3.63, p = .0144),表示心理彈性顯著提升;在VQ總分方面亦顯著提升(M = +6.84, p = .0003),反映價值行動投入增加。相較之下,等待組在相同時間內AAQ-II得分反而顯著上升(M = +2.50, p = .0205),顯示心理僵化加劇;VQ總分則呈現輕微下降(M = -1.57, p = .5199)。兩組變化量之差異均達統計顯著水準,分別為AAQ-II(p = .0008)、VQ(p = .0067),顯示ACT團體介入在提升心理彈性與價值導向生活面向具顯著成效。
本研究提供實證支持ACT團體介入能有效協助失智症家庭照顧者提升心理彈性,並增強價值導向行動,具臨床應用與政策推廣之可行性。 With the rapid growth of the aging population and the increasing prevalence of dementia, family caregivers of individuals with dementia are facing mounting emotional distress and mental health challenges. This study aimed to examine the effect of a group counseling intervention based on Acceptance and Commitment Therapy (ACT) for enhancing psychological flexibility and value-driven action among family caregivers of people with dementia.
A multiple-baseline across groups design was employed, involving 19 family caregivers who participated in a five-week ACT-based group intervention, delivered once per week. The intervention incorporated core elements from the World Health Organization's (WHO) Doing What Matters in Times of Stress guide. Outcome measures included the Acceptance and Action Questionnaire-II (AAQ-II), Valuing Questionnaire (VQ), Patient Health Questionnaire-9 (PHQ-9), and Generalized Anxiety Disorder-7 (GAD-7), administered pre- and post-intervention to assess changes in psychological flexibility and value-based behavior.
Results indicated a significant decrease in AAQ-II scores in the intervention group (M = -3.63, p = .0144), reflecting an improvement in psychological flexibility, and a significant increase in VQ scores (M = +6.84, p = .0003), indicating greater engagement in valued actions. In contrast, the waitlist control group showed a significant increase in AAQ-II scores (M = +2.50, p = .0205), suggesting increased psychological inflexibility, and a slight decrease in VQ scores (M = -1.57, p = .5199). The differences in change scores between groups were statistically significant for both the AAQ-II (p = .0008) and VQ (p = .0067), demonstrating the efficacy of the ACT group intervention in promoting psychological flexibility and value-oriented living.
This study provides empirical support for the clinical and practical utility of ACT-based group interventions in improving psychological flexibility and enhancing value-driven behavior among family caregivers of individuals with dementia. |
