    題名: 台灣社區精神復健模式與精障者復元狀況之探究

    摘要: 心理健康對於健康至為關鍵,而台灣過去十年慢性精神病患的人數增加60.87%,值得關注。精神疾病易造成個人功能損害,進而遭受社會烙印而處於社會弱勢,社區精神復健服務的目的即在幫助有精神障礙的個人增強其功能,使他們能在最少的專業協助下,於他們所選擇的環境中過著成功且滿足的生活。當今復元已成為精神復健的願景和終極目標,在西方以復元為導向的精神復健服務蔚為風潮。台灣於過去十年來社區復健中心的數量增為74家(4.87倍),然而關於這些復健中心之服務模式,截至目前仍欠缺大規模之研究,這些機構提供了哪些服務、如何提供?主管和專業人員之復元才能程度為何?又學員(接受復健者)之復元狀況為何?目前仍未有系統性的認識和了解。因此筆者預計以三年時間針對非醫院經營之社區精神復健中心加以探究,研究中將兼採量化和質化取向,首先以一年半的時間進行調查研究,檢視:1.接受復健之學員的復元程度與復元之相關因素、2.專業人員之促進復元的專業才能程度與影響此才能之因素、3.社區復健機構之組織概況和服務項目和內涵。後面的一年半將選取不同復元取向程度之機構以質化方法進行深度訪談和觀察,目的在探索:1.學員在機構接受復健之期待和實際的經驗與體驗,以及復健對其復元過程之影響;2.機構主管和專業人員對於精神復健抱持之價值和原則,對於學員在機構參與復健之期待和實際的處遇作為,由此反映其復元取向的才能狀態;3.透過參與觀察,了解機構之運作方式和結構安排,以及專業人員和學員之互動內涵。本研究以大規模的樣本檢驗復元統合模式,並以符號互動論引導質化研究之進行;研究結果將具有理論驗證和實務參考意義。
    Mental health is fundamental to health. It is noteworthy that over the past decade the number of persons with chronic mental illness (consumer) has increased by 60.87% in Taiwan. Mental illness usually causes cognitive impairment and dysfunction. Thus, consumers frequently face social stigma and become disadvantaged in society. As a remedy, psychiatric rehabilitation programs aim to enhance the functioning of consumers in order for them to live a satisfactory life in the community with the least assistance. Nowadays recovery has been the vision of psychiatric rehabilitation and recovery oriented services has become a trend in the West. During the past decade, the number of community psychiatric rehabilitation center has increased to 74 agencies (4.87 times). However, the knowledge concerning the modality of these agencies is lacking. Therefore, the investigator proposes a three-year study to tackle on this issue. In terms of methodology, this study will adopt both quantitative and qualitative approaches to fully capture the diversity and depth of the phenomenon. For the first one and half year, the investigation will include three parts: 1.To examine the extent of recovery among consumers and the correlates of recovery; 2. to examine the extent of recovery-promoting professional competencies (RPPC) among professions and the correlates of RPPC; 3. to gather the information concerning the organization, programs and services of these agencies. For the second one and half year, the investigator will adopt a qualitative approach to explore the following: 1. The expectations that consumers set forth for their rehabilitation, their experiences and feelings about the interaction and services, and how the process affects their recovery; 2. the values and principles held by the leaders and professionals, their expectation towards consumers’ rehabilitation, and how the services are actually provided; 3. to capture in greater depth on the operation and structure of the services and the nature of interactions between consumers and professions. This study aims to further test the Unity Model of Recovery on a larger sample size, and utilizes symbolic interactionism as the methodology for the qualitative study. Thus, this study would have both theoretical importance and implications for practitioners.
