    Title: 國軍維持健康體態的動機-以A軍校學員為例

    Abstract: 中華民國國軍在健康管理方面,官兵可透過年度體檢,得知各項身體健康指數是否異常,軍醫局及單位後勤部門管制指數異常人員定期回國軍醫院複診追蹤,以維護官兵健康並保持國軍最佳戰力。肥胖症是國軍最常見之定期回診項目,而國軍在減重治療主要以非藥物治療為主,透過飲食、運動、睡眠等健康行為,讓肥胖官兵體重控制在國軍定義之非肥胖體重範圍(BMI<32),達到標準後再轉由個人健康管理。據國健署(2023)統計資料,18歲以上國人高達五成機率罹患肥胖,國軍刻正面對同樣的問題,肥胖症會引發三高(高血壓、高血脂、糖尿病)之外,也會高風險造成心血管疾病及癌症。作者認為國軍在部隊健康管理方面,主要針對肥胖官兵族群予以協助,並無法有效遏止肥胖官兵人數的增加,且耗費部隊大量人力、時間及金錢成本。
    本研究係透過訪談法針對A軍校學員(本研究所提到之軍校學員為任官10年以上的少校軍官)的個人健康管理方式及動機分析,就訪談內容延伸出可能影響之行為,量化成問卷後蒐集數據,重新檢驗學員是否因各項動機影響到健康行為。研究結果指出學員在維持健康體態的動機因素,動機強弱由高到低分別為自身健康、家庭因素、同事影響、長官命令、個人升遷等5項,學員在各項減重之動機比較下,對於各項健康行為之影響無顯著差異。若依性別或年資分類樣本,研究發現各集群在家庭動機因素、每週運動頻率上將有顯著差異;另外家庭因素與升遷兩項動機相比,對於BMI過重及肥胖者有相當之影響,學員在選擇動機上,家庭因素動機若重於升遷動機,則BMI呈現負相關;學員在健康行為(飲食、運動、睡眠)表現上,不會因特定動機影響而造成顯著差異。最後,任何學員隨年紀增加,BMI值均是正成長(樣本整體每年平均BMI估計增加約為0.215 kg/m²)。
    The Taiwan (ROC) Armed Forces implements health management measures for its personnel, allowing officers and soldiers to understand their health indicators through annual physical examinations. The Military Medical Bureau and unit logistics departments monitor individuals with abnormal health indicators, requiring them to undergo follow-up consultations at military hospitals regularly to maintain their health and ensure the optimal combat readiness of the armed forces. Obesity is the most common reason for periodic follow-up visits among military personnel. Weight loss treatment within the armed forces primarily involves non-pharmaceutical approaches, emphasizing healthy behaviors such as diet, exercise, and sleep. These measures aim to help obese personnel maintain their weight within the military-defined non-obese range (BMI < 32). Once the standard is achieved, individual health management takes over. According to the Health Promotion Administration (2023), individuals aged 18 and above in Taiwan have a 50% probability of being obese. The armed forces face similar challenges, as obesity not only leads to conditions such as hypertension, hyperlipidemia, and diabetes but also increases the risk of cardiovascular diseases and cancer. The author believes that the military’s health management efforts primarily focus on assisting obese personnel but fail to effectively curb the increasing number of obese individuals. Additionally, these measures consume significant manpower, time, and financial resources.
    This study employs interviews with cadets from “Military Academy A” (specifically, majors who have served for over 10 years) to analyze their personal health management practices and motivations. Insights from the interviews were used to identify behavioral influences, which were then quantified into questionnaires to collect data and examine whether motivations impacted health behaviors. The findings indicate five motivational factors, ranked from highest to lowest, influencing cadets’ efforts to maintain a healthy physique: personal health, family factors, peer influence, superior officers’ orders, and career advancement. When comparing weight loss motivations, no significant differences were observed in their impact on various health behaviors. However, when samples were categorized by gender or years of service, significant differences emerged in family motivation factors and weekly exercise frequency. Furthermore, when comparing family and career advancement motivations, both significantly affected overweight and obese individuals’ BMI. A stronger family motivation relative to career advancement motivation was negatively correlated with BMI. Cadets’ health behaviors (diet, exercise, and sleep) showed no significant differences attributable to specific motivations. Finally, BMI values exhibited a positive trend with age, with the sample group’s BMI increasing by an estimated average of 0.215 kg/m² annually.
