    Title: 從人口老化論我國老年長期照護保險之規劃

    Abstract: 　　本研究主要是從人口結構與疾病結構的改變，來討論未來長期照護的需求情形，並且從國內家庭體系是否會因人口結構的改變與社會環境的變遷；及長期照護的問題是否會對家庭的經濟與心理上造成沈重的負擔兩方面作探討。根據研究結果認為家庭所能提供的看護能力在未來將降低，必須仰賴社會資源的協助。因此本研究針對國內長期照護保險之規劃與實施提出見解，研究結果認為根據歐美先進國家之經驗及趨勢來看，政府若將長期照護納入全民健康保險，對政府與民間的衝擊將會很大，而且也與國際潮流背道而馳，因此不贊成將長期照護納入全民健保的給付範圍內。有鑑於此，筆者試圖對長期照護保險之實施，利用社會保險、商業保險及風險分擔的觀念，提出長期照護保險之體系與架構；及三層式的長期照護保險。對長期照護保險的實施，從保險的角度切入，就保險的觀點給予建議及意見，希望能達到政府、社會、家庭與個人間的照護責任層次化，使未來老年人口獲得照護的可近性、有品質的照護及多元化的照護選擇。
    　　This paper is to study the future demand of long-term care insurance for the elderly in Taiwan, based on the following changes: population transition, disease transition, family transition, and the change of social environment. The ability of care provided by families is likely to reduce in the future, but relying sloely on social resource is not feasibile either.
