    Title: 自閉症類群障礙幼兒綜合性早期療育之研究
    Comprehensive Early Intervention for Young Children with Autism Spectrum Disorder

    Abstract: 自閉症類群障礙(autism spectrum disorder, ASD)患者在發展過程中表現出溝通、社交缺陷及行為、興趣上有重複侷限的障礙,此障礙被認為是腦部神經發展異常所致,為終其一生的問題;但透過適當的綜合性早期介入(comprehensive early intervention),可以有效地促進ASD患者在各主要發展領域的長期發展,如提昇其認知、語言及適應功能表現,進而增進患者及其家庭的生活品質及生命素質。目前國外已進行許多具實證基礎的ASD綜合性早期療育介入療效研究,國內則缺乏綜合性早期療育的研究資料。本研究擬以三年時間進行符合本國國情並與國際接軌之綜合性早療介入療效研究,研究目的分別如下:
    (一) 本研究嘗試與早療醫療院所合作,建立使用結合行為介入與發展介入優點的ESDM(Early Start Denver Model, ESDM;Rogers & Dawson, 2010)介入原則,採低密度之個別介入,並且合併家長訓練模式。
    (二) 招募2至3歲高、低功能ASD幼兒進行早療介入,在介入前、介入6個月、介入12個月、介入後6個月追蹤等四個時間點收集相關測量資料,分析介入前後測量資料以檢視介入效果。
    (三)檢視與療效相關之ASD兒童個別行為變項及其他相關變項。。本研究預計招募64名2至3歲的ASD兒童及其家長,採用個案控制研究設計,比較32名介入組(ESDM個別介入與家長訓練組)與32名控制組(一般早療介入)的高、低功能ASD兒童在介入前後的進展差異(包含認知、語言、社會溝通、適應行為、症狀嚴重度),並嘗試找出可預期介入效果的療效指標(如介入前認知能力、氣質、感覺處理、物品使用、模仿能力、眼神注視行為等)。預期本研究能建立一個合乎經濟效益及具實徵療效基礎的本土ASD綜合性早期療育模式。
    Autism spectrum disorder (ASD) was characterized qualitatively impairments on the communication and social interaction, and manifested repetitive and stereotyped behaviors and interests. These difficulties were shown in their life span with various severities. Comprehensive early intervention for the children with ASD has been reported not only to improve their cognitive, language and adaptive functions, but also facilitate their life quality and decrease the family burden. In the West, there have been developed some of evidence-based comprehensive programs of early intervention (EI) for the children with ASD. However, there were no empirical data to focus the issue in Taiwan.
    Three-year proposal is provided and the purposes are as follow:
    (1) To develop a low intensity, individual and combined parent coaching EI model by using the principles of Early Start Denver Model (ESDM) for young children with ASD.
    (2) To evaluate the effectiveness of EI model as described above.
    (3) To investigate the predictive factors of EI effectiveness.
    The study will use pre-posttest case control design and recruit 64 2-3-year-old children with ASD including 32 ASD children as the ESDM intervention and the other 32 ASD children who receive general intervention in hospital as the controlled group. Number of high and low functioning children with ASD will be recruited equally in each group. Pre- and- posttest measures at 4 time points: pre-intervention, receiving 6 months intervention, receiving 12 months intervention and 6 months post-intervention follow-up. The measures will be included cognitive function, language, social adaptation, and severity of symptomology. Cognitive function, temperament, sensory processing, object use, imitation and looking type will used as the early differential indicators in the study.
    It is expected this study can built an efficiently evidence-based comprehensive EI with international perspective for children with ASD in Taiwan.
