Abstract: | 在自殺率偏高的台灣,對自殺者的了解及自殺防治至關重要,而內在辯論是自殺者常見且重要的心理狀態。本研究使用台灣安心專線來電文字謄本作為研究資料,以「不能死的理由」取代過去研究中「想活的理由」,探討安心專線來電者的特性、想死/不能死的理由與自殺風險程度的關聯以及自殺的內在辯論。本研究之資料來源為2013-2018期間第一次來電者的電話錄音檔及台灣eSOS自殺危機個案處理系統資料庫,這些錄音檔中,依據無自殺意念、偶有自殺意念、常有自殺意念、自殺行為準備中、自殺行為進行中,從中隨機抽取150份,去除個資後將語音轉錄為文字謄本,以及轉錄與之對應的背景變項。自殺危險程度則透過兩名臨床心理學學者專家經討論獲取共識後,針對文本謄本進行分類,最後再歸類成無自殺意念組、有自殺意念組及自殺準備/行動組。想死/不能死的理由則重新編寫評分手冊,評分者間信度為.92。本研究以相關、共變異數分析及邏輯斯迴歸進行資料分析。研究發現,不能死的理由可以削減想死的理由而降低自殺風險,且想死/不能死的理由相減之內在辯論可以區分三組不同的自殺風險程度,於這些樣本中,較常出現的想死的理由為逃避、與他人的關係有困擾或不想造成他人負擔、無望感與經濟困擾,而具有對他人的責任、尋求協助、家庭、擔心造成他人困擾及死亡焦慮則是較常出現的不能死的理由。本研究亦討論研究結果與過去自殺理論的關聯以及實務意涵。 Since suicide mortality rate is relatively higher in Taiwan, understanding of suicide and suicide prevention is even more important, and internal debate is a common and essential psychological state of suicidal people. This study adopts Taiwan An Hsin hotline’s phone call transcriptions as research data, replacing “reasons for living” with “reasons for not to die” to explore characteristics of An Hsin hotline callers, relationship between reasons for dying/not to die and suicide risk, and the internal debate of suicide. This study randomly selected 150 first-time caller’s recordings from An Hsin Hotline’s suicidal follow-up case record database (eSOS) between 2013 and 2018, including no suicide ideation, seldom having suicide ideation, usually having suicide ideation, preparing for suicide attempt and attempting suicide. These recordings were transcribed into text after removing personal information and combined with corresponding background variables. Suicide risk level is evaluated by two clinical psychology experts, then categorized into no suicide ideation group, suicide ideation group and preparing/attempting suicide group. Interrater reliability of the coding manual of reasons for dying/not to die is .92, other data analyses were conducted by using correlation, analysis of covariance (ANCOVA) and logistic regression. Study shows that reasons for not to die can reduce reasons for dying, therefore suicide risk is decreased. The internal debate represented by subtracting reasons for not to die from reasons for dying can differentiate three suicide groups. Among these samples, escape, relationship with others, hopelessness and financial hardships are the common reasons for dying. Responsibility to others, seeking help, family, burdening others and death anxiety are the common reasons for not to die. Relationship between study result and previous suicide theories and implications were also discussed. |
