    摘要: C級巷弄長照站係我國長期照顧十年計畫2.0中社區整體照顧服務體系之一環,期望透過布建綿密的社區據點,落實在地老化及初級預防。然而當今研究對於C級巷弄長照站服務使用者獲益瞭解甚少,故本研究以服務使用者觀點視角,探討參與高雄市C級巷弄長照站(據點C)長者之友誼、孤獨感及生活滿意度。
    本研究之研究目的為:(1)瞭解長者在巷弄長照站參加活動或使用服務情形;(2)探討長者活動參與及使用服務後對於友誼建立之影響為何?(3)分析長者參與巷弄長照站之孤獨感及生活滿意度;(4)長者參與巷弄長照站之孤獨感及生活滿意度是否會因其個人人口特質、友誼、活動參與及使用服務而有異?(5)依據研究結果針對巷弄長照站現行概況提出政策建議。
    本研究由研究者擔任訪員,以面訪形式收集15間據點C共計209份有效問卷,研究結果發現,高雄市C級巷弄長照站服務使用者以女性、70歲至79歲、經濟及健康狀況佳者為主;最常參與或使用的活動及服務為健康促進、社交活動及技能學習;且超過九成有建立友誼並擁有知心好友;整體孤獨感低、生活滿意度高。最重要的是,本研究發現活動相較於服務能帶來更多的友誼增進,此外,探究服務使用者對於友誼滿意度的主觀評量指標-「友誼品質」係影響孤獨感及生活滿意度高低的重要因素。
    依據本研究之結果提出以下四點建議:(1)鼓勵長者積極參與C級巷弄長照站;(2)善用活動參與為長者所帶來之友誼增進;(3)積極落實促進友誼之方法,以利提升長者獲益;(4)重視C級巷弄長照站社會工作者及專業人員之專業能力。
    Tier C-long-term care (LTC) stations around the blocks are a component of Taiwan’s LTC 2.0. It is envisioned as a crucial element in the comprehensive community care service system, aiming to actualize “aging in place” and implement “upstream prevention to delay disability” by establishing dense community bases. However, there is limited research on the benefits experienced by service users of Tier C. This study adopts a service user perspective to investigate the aspects of friendship, loneliness, and life satisfaction.
    The objectives of this study are as follows: (1) To comprehend the elderly’s engagement in activities or utilization of services at Tier C; (2) To explore the impact of the elderly’s participation in activities and use of services on the development of friendships; (3) To analyze the loneliness and life satisfaction of the elderly participating in Tier C; (4) To examine whether the loneliness and life satisfaction of the elderly at Tier C are influenced by personal demographic characteristics, friendships, and activity participation, and if these differ based on service usage; (5) To provide policy recommendations based on the research findings and the current state of Tier C.
    The researcher served as the interviewer and collected a total of 209 valid questionnaires through face-to-face interviews conducted at 15 Tier C locations. Results indicate that Tier C service users in Kaohsiung City are predominantly female, aged 70 to 79, economically stable, and mostly in good health. The most frequently participated activities and services include health promotion, social activities, and skill learning. Over 90% have established friendships and have close friends, experiencing low overall loneliness and high life satisfaction. Significantly, this study reveals that activities contribute more to enhancing friendships than services. Furthermore, it investigates whether “friendship quality”, a subjective evaluation index of friendship satisfaction among service users, influences loneliness and life satisfaction, identifying it as a crucial factor.
    Based on the study's outcomes, four suggestions are proposed: (1) Encourage active participation of the elderly in Tier C; (2) Leverage the friendship enhancements resulting from activity participation; (3) Actively implement and promote methods to enhance friendship benefits for the elderly; (4) Pay attention to the professional capabilities of social workers and professionals at Tier C.
