Abstract: | 酒癮問題在世界各國均是重要的心理與社會議題(SAMHSA, 2014;WHO, 2018;Willliams et al., 1991)。研究者多年參與酒癮治療工作,深刻體認到酒癮患者本身係心理困擾的受害者,在他們的生命經驗中,總不乏破碎的家庭經驗、不良的親子關係、動盪的早年生活以及人生歷程中的失敗與失落事件,在許多研究報告中也陸續指出早年的創傷經驗與成年期的酒精依賴與濫用有密切關係(Coskunpinar and Cyders, 2013; Mills et al., 2006; Pietrzak et al., 2011; Pine and Cohen, 2002; Stautz & Cooper, 2013;葉美玉和李選,1996;楊美賞等人, 1995)。然而,對於酒癮患者的治療仍多停留在生物醫學與認知行為層面的探討,對於心理動力層面的研究相對顯得稀少,相關的療效研究更是闕如(Nady el-Guebaly, Giuseppe Carrà, Marc Galanter, 2015)。因此,本研究擬從酒癮患者生命經驗的探討開始,了解酒癮患者成癮的心理歷程,辨認其中最為關鍵的早期創傷與自我棄逐(消沉)經驗(第一年研究) ,探究其早年逆境經驗、消沉歷程與其酒癮行為之關聯,並進一步提供諮商協助,探索其諮商歷程所呈現的晤談感受、個人期待與探索經驗、諮商關係變化以及覺察早期經驗對個人成長的獲益等方面,了解酒癮患者的心理困境與其治療策略,藉以探討酒癮患者初期諮商歷程之變化與獲益因素(第二年研究)。 Alcoholism is an important psychological and social issue in countries around the world (SAMHSA, 2014; WHO, 2018; Willliams et al., 1991). Researchers have been involved in the treatment of alcohol addiction for many years and deeply realized that patients with alcohol addiction themselves are victims of psychological distress. In their life experience, there is always a lot of broken family experience, terrible parent-child relationships, turbulent early life. Many researches report that early trauma experience is closely related to adult alcohol dependence and abuse (Coskunpinar and Cyders, 2013; Mills et al., 2006; Pietrzak et al., 2011 Pine and Cohen, 2002; Stautz & Cooper, 2013; Ye Meiyu and Li Xuan, 1996; Yang Meishu et al., 1995). However, the treatment of patients with alcoholism is still mostly at the level of biomedicine and cognitive behavior, and the research on psychological dynamics is relatively scarce. (Nady el-Guebaly, Giuseppe Carrà, Marc Galanter, 2015). Therefore, this study intends to start with the exploration of the life experience of alcohol addiction patients, understand the formation process of alcohol addiction patients, identify the most critical early trauma and self-affliction experience (first year research). In the further, we provide the services of counseling to interviewees. Then invites them to the next stage of this study to explore the experience of interviews, personal expectations and exploration experience, changes in counseling relationships and awareness of the benefits of personal growth. The purpose of this stage is to understand the psychological dilemma of patients with alcoholism and their treatment strategies, in order to explore the changes and benefits of the initial counseling process of patients with alcoholism (second year research). |
