Abstract: | 2020年COVID-19震驚了所有國家的經濟和衛生系統,國際主要財經機構紛紛大幅下修全球經濟成長率,造成供應鏈受阻進而影響生產等「蝴蝶效應」,且衝擊民間消費,來到了2021年全球疫情至今仍未有效解決,然而對於經濟的衝擊已隨著各國陸續接種疫苗而逐漸慢慢淡化,全球經濟可望回到正常成長軌道。每當經濟發生動盪時,所有的產業或多或少都會受到影響,唯獨醫療不受影響,這次的疫情則因防疫政策實施減少不必要醫療行為以及延後非緊急之醫療手術,受影響範圍大幅擴大至醫療產業,但據USRDS統計,這二年透析人次有大幅增加現象,此異常於統計,於2020年度資料出爐時發現,當下研究人員詳查其原因, 確認主因在於因疫情造成重症病患併發了急性腎損傷,而增加之,為例外現象。但不論任何情況下,因透析是不可中斷的治療,原則上有國家社會保險支付者是不受任何經濟影響。
台灣血液透析市場於2005-2006 年間有很明顯的變化,即為走向集團化的經營模式,原透析儀之代理醫療器材貿易商,透過本身代理的優勢,以及專科醫師之人脈關係,與其合作,發展血液透析醫療服務一條龍商業模式,整合著透析產業,建立血液透析中心管理服務。其營運主要模式以「整合資源」方式,成為整合醫療保健之通路商,除銷售醫療產品之外,另主要業務為提供諮詢服務(除治療外),另擔任策略夥伴合約者之醫務管理顧問角色,建構醫療管理服務系統, 以「醫務管理」的模式建立著此生態系,化身為醫療院所之醫務管理顧問角色,除原業務(供應硬體設備及相關耗材)外,亦提供適於產業之管理軟體,協助建置人員職前訓練、成本分析、財稅管理以及醫療品質管理及分析等業務,以加強執業醫師的經營績效和病患就診滿意度,讓原本不太懂得商管的專科醫師們開始可獨立開設診所,分享著這個大餅。此產業在多年的成長下,已經形成透析產業生態圈,已是大吃小的情勢,形成規模經濟。
本研究採用的研究方法在廣泛收集資料的基礎上,首先對慢性腎臟病發展情況進行文獻回顧,通過資訊瞭解其發展因素及其他關聯性,且對腎臟替代療法進行剖析,並指出台灣健保現況以及病患選擇就醫之消費者心理。隨後,論文運用PEST 模型對全球透析產業環境進行分析,並依資料分析全球透析市場概況以及 產業發展現況方向及未來科技趨勢。再次運用PEST 模型對台灣透析產業環境進行分析,並用波特五力模型對台灣透析市場現況進行剖析,以及預估市場未來供需及是否仍有投資之超額利潤存在,結合已分析之產業發展技術和市場分佈,提出了台灣透析未來在不同面向上,可進行投入的建議或改善方向。
本研究認為,台灣血液透析市場已飽和,無超額利潤可以吸引新進入者,且全球腎臟治療走勢為研發新治療法,進而減少血液透析治療法的使用,以減輕其帶來的沉重社會負擔;依資料顯示部分國家施行預防政策後,有效降低新發生之成長率,但費用負擔未有明顯的效果,現在最主要是推動成本較低的居家透析取代中心血液透析,讓成本有效下降;雖說台灣血液透析市場已無投資超額利潤機會,但其週遭相關產業仍是可期待的機會,例如患者的每日飲食記錄追蹤調整。 In 2020, the pandemic we called COVID-19 had created both public health and economic crisis all over the world. With a global economic slowdown, COVID-19 had led to an international supply chain hit affecting the production line and other "butterfly effects". Nowadays, even in 2021, the pandemic had not been effectively resolved yet. However, the impact on the economy has gradually faded as countries successively vaccinated, and the global economy was getting back to normal. Whenever there is an economic crisis happened, almost all industries would be affected, just the more or the less. Among them, it’s found that the public health care industries are not affected that much. Because of the pandemic and effective usage of medical resources, people were told to be stayed at home, reduced unessential medical treatments, and postponed non-urgent surgical processes. Despite COVID-19 having also been greatly expanded to the medical industry, according to USRDS statistics, the number of dialysis visits had unexpectedly increased in the past two years. When the data in 2020 was released, researchers investigated why this industry is not impacted by the global pandemic. The reason was that acute kidney injury was caused by the epidemic as an exception, dialysis is a treatment that cannot be postponed, also recognized by the National Health Insurance to pay. During 2005-2006, the hemodialysis industry had been fast-growing and the corporate group started involved in this industry. The dialysis machine`s agents had their own agency advantages and the network of specialist physicians cooperated with the corporate group. The new business started to develop a one-stop business model for hemodialysis medical services, integrating the dialysis industry, and establishing hemodialysis center management services. The key is to integrate resources and become a distributor of integrated healthcare, not only selling machines and instruments but also providing consulting services (except treatment), acting as a medical management consultant for all strategic business partners. They build a medical management service system and provide medical management consulting services in medical institutions. In addition to the original business, supplying hardware equipment and related consumables), it also provides industry management software in the establishment of pre-employment training, cost analysis, customer relationship management, financial and tax management, and medical quality management and analysis. This enhanced the operating performance of practicing physicians and the satisfaction of patients. After years of growth, this industry has already formed a dialysis industry ecosystem with economies of scale, which is owned and operated by a corporate group. The research method is based on extensive data collection. First, a literature review is conducted on the development of chronic kidney disease, and its development factors and other correlations are understood through the information, renal replacement therapy analysis, and bias or requirements or reasons of patients taking specific medical treatment. Then, the thesis uses the PEST model to analyze the global dialysis industry, along with the market overview, current industry development direction, and future technology trends. The PEST model is also used to analyze the dialysis industry in Taiwan, and Porter`s five forces model is used to analyze the current situation of the Taiwan dialysis market. The purpose is to estimate the future supply and demand of the market and whether there is still excess profit from an investment, combined with the analyzed industry development technology and market distribution, and suggestions or improvement directions for Taiwan`s dialysis in different aspects in the future. This study believes that Taiwan`s hemodialysis market is saturated, and there is no excess profit for the new entrants. Because the global trend of renal treatment is trying to develop new treatments, thereby reducing the use of hemodialysis treatments, and reducing the heavy social burden it brings. According to the data, after CDC’s strict prevention in some countries, the growth rate of new COVID-19 occurrences has been effectively reduced, but the cost burden has not had a significant effect. Even though Taiwan the hemodialysis market has no opportunity for excess investment, it’s still worth promoting lower-cost home dialysis to replace central hemodialysis. The related industries around are still having opportunities, such as the tracking and adjustment of patients` daily dietary records. |
