    Title: 整合實證醫學與網際網路服務於心血管疾病患者之風險評估系統

    Abstract: 近年來由於世人的生活習慣與飲食型態的改變,使得心血管疾病等慢性病成為世界各國威脅民眾生命健康的重要疾病之一,在台灣更連續數年名列人民十大死亡原因的第三、四名。如何提升一般民眾瞭解自身的風險,以有效降低罹病機會及提早就醫,是醫學界共同的研究目標。本論文的目的在於希望結合實證醫學的概念,利用現有最佳的醫學實證,提供民眾正確而有效的心血管疾病風險評估,並且透過網際網路的服務,能隨時隨地進行風險評估與分析,並且獲取適當的個人化的衛教資訊。本論文的發展階段分成兩個步驟:首先由專科醫師與公衛專家針對心血管疾病風險因子的文獻進行系統性的文獻評估,評選出適合國人的風險因子及計算公式;第二階段發展出以網際網路為基礎之個人健康管理系統,提供一般民眾進行個人化的風險評估以及追蹤自我風險變化的情況。本系統將實證醫學概念直接應用於民眾身上,提供一般民眾在初期預防階段,隨時隨地瞭解自己罹患心血管疾病的風險等級,及對造成疾病發生之危險因子有正確的認知,達到有效控制心血管疾病的危險因子,使未發病的民眾及早診斷與治療,減少罹病率(morbility),協助已確診的病患減少併發症的發生,進一步降低死亡率(mortality),以期達到活得健康,活得更久的最終目的。
    Recently, cardiovascular disease (CVD) has already become one of the most critical diseases for many countries. Especially in Taiwan, the death by CVD is named as the third or forth in the top-ten reasons of death for many years. Therefore, it is significantly to educate people to understand the risk of suffering from the CVD. In this paper, we present an evidence-based medicine (EBM) based approach to assessing the risk of suffering CVD disease. There are two phases in our development: First, a systematic review of literatures of medical evidence is preceded by several professional physicians and experts. Second, a web-based assessment system that according to the reviewed results from first phase is constructed for people to evaluate their risk factor. The main contribution of our research is to integrate the EBM with the web technology for people to evaluate their risk factors of cardiovascular disease. The EBM provides the existing best medical research evidence for CVD risk factors. Next, the web technology provides the flexibility of anytime and anywhere for people to assess the risk by oneself.
